Our Athletics Program

The Mission of Tolleson Union High School District Athletics is to "create an environment where the student athletes, coaches, faculty, and staff are committed to enhancing the total student athlete experience by promoting opportunity, protecting student athlete welfare, and fostering a positive student athlete image."


Athletic Clearance Procedures

If you are interested in joining a sport, please download and print the entire athletic packet located at the side of this page. You must complete these forms, as well as the following eligibility requirements, before we allow you to try out or practice in interscholastic competitions:

  • Athletic Packet Forms 
    • Consent for Emergency Care
    • Annual Preparticipation Physical Evaluation
    • Annual Preparticipation Physical Examination 
      A licensed M.D., D.O., N.P., or P.A. must complete and sign the form. Physicals taken on or after March 1st of this year are good for the following school year.
    • Domicile Form
    • Equipment Check Out Form
  • Proof of Medical Insurance
  • A Birth Certificate
  • An annual athletic fee of $50 per sport is due at the beginning of the season. This is payable at the student bookstore.
  • Athletic Statement of Awareness Form
  • NCAA Clearinghouse Information Sheet
  • AIA Brain Book Concussion Course (must complete and turn in certificate to athletic office)
  • Please watch the Informed Consent Video with your child prior to beginning any  sport.

Please bring completed forms to the athletic secretary at least one week prior to try-outs or practice to receive a clearance slip. Please do not give the forms to the coach or trainer.
